Understanding HRT

March 2026Hormones & HRTMedically reviewed

Hormone Replacement Therapy (HRT) involves taking hormones to replace those your body no longer produces in sufficient quantities during menopause. It is the most effective treatment for vasomotor symptoms (hot flashes and night sweats) and can also help with mood, sleep, vaginal dryness, and bone health.

Types of HRT

There are several types of HRT: Estrogen-only HRT — for women who have had a hysterectomy. Combined HRT (estrogen + progesterone) — for women with a uterus, as progesterone protects the uterine lining. Sequential/cyclic HRT — progesterone is taken for part of the cycle, producing a monthly withdrawal bleed. Continuous combined HRT — both hormones taken daily with no planned bleeding. Some women may also benefit from low-dose testosterone for libido and energy.

Key facts at a glance

  • HRT replaces declining hormones to relieve menopause symptoms
  • Available as pills, patches, gels, creams, and more
  • Can reduce hot flashes by up to 75%
  • Discuss risks and benefits with your healthcare provider

Delivery methods

HRT comes in many forms: oral tablets (pills), transdermal patches, topical gels and creams, vaginal rings and inserts, injections, pellets (implants), lozenges (troches), and nasal sprays. Transdermal methods (patches, gels) may carry lower risks than oral forms as they bypass the liver.

Benefits

HRT can significantly reduce or eliminate hot flashes and night sweats, improve sleep quality, stabilize mood and reduce anxiety, relieve vaginal dryness and discomfort, protect bone density and reduce fracture risk, and may improve cognitive function in early menopause.

Discussing with your doctor

The decision to start HRT is individual. Discuss your symptoms, medical history, family history, and personal preferences with your provider. Modern evidence supports that for most women under 60 or within 10 years of menopause onset, the benefits of HRT outweigh the risks.
